J Arthroplasty. 2018 Jul;33(7S):S13-S18. doi: 10.1016/j.arth.2018.03.01322 patients with a BMI over 35 and scheduled for total knee arthroplasty were randomized to either intraosseous regional administration (IORA) vancomycin 500mg or intravenous vancomycin 15mg/kg. Results demonstrated significantly greater concentrations of vancomycin in the IORA group compared to the IV group in both subcutaneous fat samples and femoral bone samples.
