Multimodal surgical site injection has no clinical benefit versus control in ankle fracture fixation

Patients undergoing fracture fixation are at risk for experiencing severe pain and requiring opioids. Given the ongoing opioid crisis, any intervention that can reduce the reliance in opioids is of interest. The authors of this study randomized 100 patients undergoing surgical management of isolated, closed ankle fractures to receive a multimodal surgical site injection or no injection (control). They found that while the injection reduced pain in a statistically significant manner up to 48 hours, it had no impact on opioid use, length of stay, or discharge destination. As well, the improvement in pain scores did not reach clinical significance.
