450 patients with Garden grade 3 or 4 displaced fractures of the femoral neck were randomized to be managed with total or hemiarthroplasty or internal fixation, to compare long-term failure and patient-reported outcome between the two groups. Pain, mobility, and mortality rate were similar between the two treatment groups.
