Autografts versus allografts for anterior cruciate ligament reconstruction

8 meta-analyses which compared autografts and allografts in anterior cruciate ligament (ACL) reconstruction were included in this systematic review. The purpose of this study was to determine if there was any difference in outcome between graft choices when considering re-rupture rate, clinical outcome, or measures of knee stability. Half of the included studies reported lower re-rupture with autograft compared to allograft, with mixed findings in clinical outcomes and knee stability measures. However, studies showing a difference in re-rupture rates were of lower quality, so results are inconclusive. Additional randomized controlled trials are needed to more clearly compare autografts and allografts in ACL reconstruction.
