OTA 2015: Close contact casting vs. internal fixation in treatment of malleolar fractures

620 patients with a malleolar fracture were randomized to receive either close contact casting (CCC) or open reduction and internal fixation (ORIF) as treatment. The purpose of the study was to compare functional and clinical outcomes, as well as costs, between the two groups. Results indicated that CCC provided similar clinical and functional outcome to that of ORIF at 6 months post-treatment, with significantly lower costs associated with nonoperative management through casting.
