10 randomized controlled trials were included in this meta-analysis and systematic review. The purpose of this study was to compare patient-specific instrumentation (PSI) and conventional instrumentation in total knee arthroplasty for outcome parameters of component alignment and operative time. Results of pooled analyses demonstrated no significant differences in accurate restoration of the mechanical axis, or in the incidence of outliers of the femoral component in either the coronal, sagittal, or transverse planes. PSI was associated with a significant higher incidence of outliers of the tibial component in both the coronal and sagittal planes.
